Supported Living Program
The Supported Living Program provides individual intervention and support services twenty-four (24) hours per day. Groups of two or three individuals live in a residence with an on-site instructor. Training is provided in personal and social skill areas as identified by a comprehensive assessment and the Individual Service Plan. Community integration, independent living skills, health and safety, communications, physical fitness, and education of client rights and advocacy are areas that are covered in the personal and social skill training. Support services including generic community resources, are provided as necessary. Living expenses are paid by the persons served from their Supplemental Security Income and other personal funds.
Independent Living Program
The Independent Living Program provides services to individuals requiring staff support of less than twenty-four (24) hours per day. These programs do not have staff residing in the residence. Services include residential objectives as stated in their Individual Service Plan. training, independent community living skills, housekeeping and homeowner skills as well as client rights and advocacy support services including generic community resources. Living expenses are paid by the individual from their Supplemental Security Income and other personal funds.
Respite Care
The Respite Service is a flexible family support service. The primary purpose of respite is to provide support to the individual and provide the time away from their caregiver duties.
Scope of Service
- Provide routine health care (e.g., toileting)
- Assist the individual to enhance self-help skills, leisure time skills and community and social awareness.
- Prepare or assist the individual in meal preparation and eating.
- Provide opportunities for community and neighborhood integration and involvement.
- Provide opportunities for leisure, play and other recreational activities.
- Provide opportunities for the individual to make his/her own choices with regard to daily activities.
Service Requirements
- Respite Care Services are available to any individual of any age. Respite Services maybe accessed by individuals who receive home based services but not by individuals who receive other Community Living Services.
Location requirements for Respite Care Services.
- The individual's home or provider's home.
- A community setting of the family's choice (e.g. community center, swimming pool, park).
- A center in which other individuals are provided care.
The staff to individual ratio is typically 1:1 for the period of time in which an individual is receiving respite services. Service provision in a group is permissible when appropriate to the individual, however; a staff to individual ratio may not exceed 1:4.
Community Access Services include:
- The Identification of vocational goals and objectives
- The direct involvement and the informed choices of the persons served.
- The necessary supports needed to achieve and maintain vocational goals.

- Providing Meaningful Days. This individualizes the Day Habilitation Program and makes it a more client-centered realm of activity.
Individual Supported Employment
Persons who want to work in competitive jobs in the community are trained at the work-site after they have been hired and given all the benefits of a non-disabled individual. A job coach works with the new employee until they learn and perform the job to the employer's satisfaction. This program was made possible by an amendment to the Rehabilitation Act of 1973 through Public Law 99-506 (Rehabilitation Act of 1986).
Other supported employment services include assistance with social security and other public benefits, transportation and other ongoing natural supports.

Participants gain social skills, motor skills, language skills, camping and travel skills, and group sharing skills. They also gain a sense of independence as they explore their community and surrounding areas without the aide of parents or other family members. They gain the satisfaction of knowing that they, too, are capable of learning new skills, making new friends, and taking care of their own basic needs. Their lives are enriched through visits to museums, theater productions, athletic events, educational exhibits, amusement parks, and much more. They become recognized as productive, significant human beings entitled to all of life's pleasures.
